Monitoring Form

Part of Case Management Standalone Suite

The Monitoring Form captures information on required monitoring activities, reviewing the current circumstances of the person, satisfaction with their plan, progress towards outcomes. The Monitoring Form  also addresses any concerns the person may have about their choice and rights, runs through questions related to health and welfare, review of the current back-up plan and emergency plan, incidents, referrals, recommended actions, and attempted contacts. 

Here's How:

  1. Login
  2. From the eChart or Dashboard, click Monitoring Form
  3. Click the green Add button at the top right.
  4. Complete the Section 1 and click Save. More sections, up to 10, will appear to be filled out.
  5. Use the Edit and Save buttons to work on each section. Some sections have an Add button to add multiple records of information.
  6. When completed, there is an option to Print the form.

Related Settings:

In Admin Tools -> Case Management Template, the Monitoring Form module can be Linked. This is so the user can have a task appear in their My Work list to complete the Monitoring Form as part of the Case Management Plan. The My Work task will take the Case Manager right to the Monitoring Form  module to complete the task when it is due.
