Case Management Module

Part of Case Management Standalone Suite

The Case Management Module was created to track activities that are required to support a person throughout their time of receiving case management services. Specifically, this module can be used to manage tasks that are required outside of the regular Person-Centered Planning process; these items are handled in the PCP module.

Here's How:

  1. Login
  2. From the eChart, the Case Manager can click on Case Management to open the board, which displays a list of case management plans. 
  3. If this is your first time going to this screen, Click on Start Case Management.
  4. From the Start Case Management pop-up, Select the Case Management Template which is the most appropriate plan for the person.
  5. Click Save.
  6. Once the appropriate template is selected and associated with the person, the case management tasks start appearing due on the Case Management board, as well as in the staff’s My Work. 
  7. Staff will work through the tasks, either from the Case Management board, or My Work, Marking Tasks as Done and adding any notes or files as required with each activity. For linked modules, the user will be taken right into that module to complete the activity.
  8. When all tasks are completed, and / or it’s time to start a new Case Management Plan, staff can click the green Case Management Tasks Completed, which will close out the Case Management Plan. A new Plan can be created, selecting a different template as appropriate, starting back at Step 2.
