SIP: How to create a Service Implementation Plan
We are excited to share the new DDA required SIP-Service Implementation Plan. Let's look at this new easy to use document.
- Go into the eChart and then the PCP button.
- Begin by starting a draft PCP (see How to Complete a PCP)
- Enter the Outcomes, Goals and Strategies in the PCP module.
- Once you are ready you can click on the SIP button from the PCP list view
- Next you will click on Add SIP.
- Make sure to select the correct program when entering the Basic Information and click Save.
- Next you move to Goals and Outcomes. Here you can double check what you had entered into the PCP.
- In the next section you can add any additional information.
- If you are satisfied with your document click on Generate
- You can then View/Sign and Print this document.
Quick Tips:
- All work is done in the PCP module
- Update your current draft Outcomes, Goals and Strategies to fill out the new sections
- Generate this document before you implement your PCP to make sure everything is as you want it to appear
- You can generate several SIP's prior to implementation to capture any changes you may have been asked to make
- Once you implement the PCP the SIP and PIP are locked
- The PIP will still be available for you to use as an addendum to your SIP
