ABC Data Form

The ABC Form is a report that staff can complete when someone they support has a behavioral incident that requires a detailed report of the antecedent, behavior, and consequence. The ABC Data Form can be found on a person’s eChart. Let's take a look.

Step 1: When you click on ABC Data Form, you will see a list of all the ABC’s on file for the person. You can filter these by program and/or timeframe. To add a new form, click the green “Add New Form” button.

Step 2: Fill out all of the questions on the form.

Step 3: Click Sign Document to Sign Document

Step 4: Now this report is showing as in progress. Depending on your role you could be responsible for adding a follow up action and you can also click to sign the document. 

Step 5: Again, based on your role and permission you can now mark this from In Progress to Mark as Complete. These next few functions will be based on your organization work flow.

Step 6: Once marked as complete it should be reviewed and then Mark as Reviewed.

Step 7: Now this document will show as completed.

Step 8: Click Print to Print the ABC form.

As with all our new features there are permissions for using this new module. System Administrators need to access permission settings and go to Clinical Services. Please make your selections for each role and be sure to save.
