Person-Centered Planning Module (Case Management Suite)

The Person-Centered Planning (PCP) module in iCM's Case Management Suite was created to track required activities that are required to support a person as part of the Person Centered Planning Process. Different work plans can be assigned through templates, which lay out the tasks the case manager must complete to keep the plan on track and in compliance.

Here's How:

  1. Login
  2. From the eChart, click on PCP to open the board, which is where PCP’s get tracked.  
  3.  If this is your first time going to this screen, click Configure PCP and enter the Annual PCP Date.
  4. Click Start PCP to Select PCP Template that would apply to the person, based on what the agency is required to provide leading up to the PCP. 
  5. Once the appropriate template is selected and associated with the person, the PCP tasks start appearing due on the PCP board, as well as in the staff’s My Work. 
  6. Staff will work through the tasks, either from the PCP board, or My Work, clicking the green Mark As Done button to complete tasks. 
  7. Optional. Within each task, the case manager can add any Comments or attach files by using the paperclip icon as required with each activity. Note when commenting that the Mentions feature is available, which allows the case manager to tag another staff in the comment by typing "@" and then that staff's name.
  8. When all tasks are completed, and / or it’s time to start a new Person Centered Plan, staff can click the green PCP Completed button.

Rules:

  1. All tasks will appear in My Work based on the settings configured in the Plan Template.
  2. Plan may not be completed until all Compulsory tasks have been completed.
