Activity Note

Part of Case Management Standalone Suite

The Activity Note module is designed to capture activities completed by the case manager. In the Activity Note, the Case Manager tracks the date and time as well as the contact type, purpose of the activity, relevant background and circumstances, activity detail, as well as any issues and next steps/ follow up plans.

Here's How:

  1. Login
  2. Click on Activity Note from the Dashboard Screen, which gives you the global view of all Activity Notes for people assigned to you. Or, you can go into the person’s eChart and click on Activity Note from there.
  3. The list view will show activities that have been completed by date, type, and who updated by/on.
  4. To add a new Activity, click the green Add Note button at the top right. 
  5. Select the individual. 
  6. Select the type of Activity. Based upon your selection, additional fields may pop up and the note will be marked as billable or non billable based on your agency’s settings. 
  7. Enter Activity Date, & start and end time. 
  8. Fill in the required fields on the Activity Note Form. 
  9. Select Status as Active.  
  10. Click the green Save button at the top right.

Rules:

  1. Cannot be entered for a future date.
  2. Cannot enter dates more than 1 year old.
  3. If Start and End time overlaps with other notes for other people supported by the same user, the user will be warned when saving about the time overlap to make a correction.
  4. Activity type “Comprehensive Assessment” can only save if the person's service category is Comprehensive Assessment.
  5. Activity type “After Date of Death” cannot be entered before Date of Death as recorded in the profile.
  6. Activity type “Work After Service End Date” must be done after Discharge Date.
  7. Activity type “Work Prior to Referral Date” cannot be done after the Referral Date in the profile.

Related Settings:

  1. Activity types can be set as billable or non-billable in Admin Tools -> iCM Settings-> Activity Note Settings.
  2. The form can be signed by turning on the signature setting in Admin Tools -> iCM Settings ->Signature Settings -> Activity Note Signature Settings.
