How to generate doctor appointment documents?

A Physician Medication Order Form (PMOF) and Medical Appointment Record can be generated from the system for people you support. A PMOF is an all in one document with medication list and spaces for new orders. A medical appointment record does not have a medication list.

Here's how:

1. Select a person you support. Go to the eChart -> Doctor Appointments.
2. Where you see the word Documents click on the word "Documents" (not the arrow).
3. Select generate and you will get a pop up window.  
4. Select the document type for the appointment (PMOF or Medical Appointment Record).
5. Next you will see a Manage Appointment Documents section right below the appointment information. Here you can upload, View and print the document.
