Form Builder allows you to create your own custom forms. This is a powerful tool that agencies can use to build their own forms and use them throughout iCareManager.
Step 1: Click on Custom Forms List in your Admin tools (wrench icon on the left navigation bar for system administrators).
This brings you to a list of all of your custom forms.
Step 2: Click the Green Form Builder button at the top right.
There are 3 types of components to help you build your document: Basic, Special and Layout
Drag and Drop form components to build your custom form.
Tip: You can define how you want each field to look and behave using this pop up:
Step 3: Once you are finished building your form, click Save Form.
Step 4: Click on Forms List to see all the forms you have built.
Step 5: On the Forms List, you can click Publish to make the form available to staff.
Step 6: When publishing a form, you can decide where it will be visible, the color of the form button, the icon, and more. You can even define permissions for which roles can use the form.
Step 7: Click Publish.
Step 8: The form becomes available on the dashboard under Custom Forms
Step 9: Click on the form you want to fill out
Step 10: Click Add Data to fill out the form
